Urban Health Plan
What: Primary and specialty medical care at five locations in the Bronx, including two in Hunts Point/Longwood.
Help completing federal insurance applications for Medicaid, Child Health Plus, Family Health Plus and PCAP Medicaid (Prenatal Care Assistance Program).
Medical services are available on a sliding fee scale. No patient is denied services based on inability to pay.
